All documents and information relating to immunization requirements, medical and mental health care services, and pharmacy delivery needs are housed within the Patient Portal. It is important that all students are familiar with this portal and how to access it. We encourage all students to sign in to the portal once they have obtained their Fisher network credentials. NYS residents: The Patient Portal is linked to immunization information from the New York State Immunization Portal. If you have lived in NYS since birth, we likely have your records. Our team will verify your information and communicate through the portal. Please check your for any communication.
All non-NYS residents: Upload your immunization record from your medical provider to the Patient Portal. Instructions for entering and uploading your information are on the .
