Immunization Requirements for Enrollment to the University
To comply with both New York State Public Health Laws (NYS PHL) Section 2165 and Section 2167 and University policies, please see the immunization requirements for enrollment detailed below.
Requirements for All Students
MMR Vaccination
- If you are a student who was born after January 1, 1957 and are enrolled in six credit hours or more, you must provide Fisher with proof of immunity to measles, mumps, and rubella.

| Mumps | One dose of live mumps vaccine (received no more than 4 days prior to 1st birthday), or Physician diagnosis, or Serologic evidence (titer). |
| Rubella | One dose of live rubella vaccine (received no more than 4 days prior to 1st birthday) or Serologic evidence (titer). Note: Clinical diagnosis is not accepted. |
Meningococcal Meningitis Vaccination
- If you are a student living on campus, you must provide proof of a Meningitis vaccine and/or a completed Meningitis B series within 10 years of enrollment. This is a condition of the Housing License.
- If you are a student living off campus (undergraduate, graduate, or doctoral) and regardless of the number of credits you have, you must provide the University with proof of a Meningitis vaccine and/or a completed Meningitis B series within 10 years of enrollment or complete the Meningitis Response Form.
Please Note:
Residential Students: This is a condition of the Housing Agreement and must be provided prior to your arrival to campus.
Commuting Students: You have 30 days after the first day of classes to submit this information. Once we reach the 30-day mark, you may be asked to remain out of classes, and a hold will be placed on your account until the necessary paperwork is submitted. Failure to meet this requirement may also result in being withdrawn from the University.
Requesting a medical or religious exemption:
- Medical Exemption: A written statement from a physician, PA, or NP specifying a valid medical contraindication, which vaccines are contraindicated, and the duration of the exemption.
- Religious Exemption: A written, signed statement from the student (or parent/guardian if under 18) explaining the objection to immunization based on sincere religious beliefs.

Uploading Immunization Information to the Patient Portal
All documents and information relating to medical and mental health care, immunizations, and pharmacy needs are housed within the . Students use their Fisher network credentials to log in to the Patient Portal to enter information and upload documents and forms as necessary.
Students can submit immunization records from high schools, transfer institutions, or health care providers.
Printing Your Immunization Record
For students and alumni enrolled after fall 2015, print your immunization records through the .
